MAKE UP DOCUMENT TURN-IN

Our make-up document turn in is scheduled for
Saturday March 11th from 10am-1pm at the Mid-Willamette Family YMCA 3201 Pacific Blvd SW Albany, OR 97321

Some clarification:

1. All players regardless of prior year participation must show a completed medical release. Yes, similar info is entered at time of registration, but the paper form is verified by the league and then given to the coach. We will have blanks available at document turn in.

2A. All players regardless of prior year participation must provide residency verification using this info from Little League International. https://www.littleleague.org/university/articles/residency-requirements/ or Little League School Enrollment Verification Form (Form A) completed by the player’s school by a member of the administration, located here. https://www.littleleague.org/downloads/school-enrollment-form/

2B. The only exception to the residence verification is if we have record of Form A being submitted in previous years and 1. The player hasn’t changed Schools and 2. The Player has not taken more than a one year break from registration with MVLL.

3. All players must show an original or certified copy of the players birth certificate to certify players age.
A. - This rule is implemented to ensure players league age for verified regular season play specifically for tournament eligibility from age 8 and older. To limit confusion, we ask that all players regardless of age, show or have shown a birth certificate at registration.
B. - Players under the age of 8 are not specifically required to show proof of age unless a discrepancy is brought to our attention.

Player evaluations have ended for ages 8yo and older. Any players 8yo and older who did not attend or who registered after evals were held, will be placed on a team using their age as the deciding factor.

Reminder: Registration ends March 11th at the conclusion of Document Turn-in (1:00pm) and no late registrations will be accepted. Players who still need to show documentation after registration ends will have that opportunity. Date/Time/Location is still TBD.
