A. Required Documents for Registration
- Medical Release ~Required for Spring Registration~ (Print and complete this form)
-Form A NEW "Form A"is an option for proof of boundary enrollment (This form is completed by parent/guardian and players school)
      The use of Form A replaces the need for Proof of Residency requirements.- Proof Of Residency and Age Requirements 
- Proof Of Residency and Age Requirements (Find here is the list of required documents if Form A does not apply to your player)
- Proof Of Age Requirements  (This is the league requirement for the players original birth certificate)

B. Registration Informational Documents
- Baseball Age Chart
- Softball Age Chart
- Form A - This form can be used in place of Proof of Residency.
- Policies and Waivers Policies and Waivers required for Registration

Boundary Exemption Forms

Little League Boundary School Enrollment Forms:
Form A - Initial Request Enrollment
Form B - Change of school status (Use this form only if Form A was submitted in previous years)
